The links to the left contain all of the necessary registration forms for the participating students and teachers. Everyone should read the Participant Letter and Daily Itinerary to understand the activities of the 4-day program.

Students:
Print, complete, and mail/fax the Student Registration, Photo Waiver, and Medical Insurance forms. Be sure that all forms are signed by a parent.

Teachers:
Submit registration forms by e-mail or FAX (no signatures required).
