The links to the left contain all of the necessary registration forms for the participating students and teachers. Everyone should read the Participant Letter to understand the activities of the 4-day program.

Students:
Print, complete, and mail/fax the Student Registration, Photo Waiver, and Medical Insurance forms. Be sure that all forms are signed by a parent. Priority will be given to students who have returned the required documentation by Friday, May 15, 2009.

Teachers:
Submit registration forms by e-mail or FAX (no signatures required). Priority will be given to teachers who have returned the required documentation by Friday, May 15, 2009.
